Data is essential as health centers attempt to balance the provision of care and recognizing how inequities impact the health of their patients and communities. Working toward health equity requires collecting information about the social determinants of health and monitoring health outcomes and disparities to identify what interventions are needed and the impact of different strategies. These resources explore existing data on health equity and provide tools that can be used in health centers.

TOOL: Enabling Services Data Collection Implementation Companion

Enabling services are non-clinical services that improve access to care, such as transportation and interpretation. It is often hard for health centers to illustrate the importance of these services.
