Data and Measures—Starter Kit


Data is essential as health centers attempt to balance the provision of care and recognizing how inequities impact the health of their patients and communities. Working toward health equity requires collecting information about the social determinants of health and monitoring health outcomes and disparities to identify what interventions are needed and the impact of different strategies. These resources explore existing data on health equity and provide tools that can be used in health centers.

RESOURCE: County Health Rankings and Roadmaps


The County Health Rankings include extensive data, by county, on many health factors and outcomes. Their model breaks down how they use social determinants to understand the current health of counties and projections of how healthy they will be in the future.

REPORT: Exploring Consensus Across Sectors for Measuring the Social Determinants of Health


This study, by Public Health Institute's Build Health Places Network, assesses the current state of SDOH measurement across sectors by systematically identifying how many and what kinds of tools exist and whether there is consensus around SDOH categories and indicators selected.

TOOL: PRAPARE Assessment Tool


The Protocol for Responding to and Assessing Patients' Assets, Risks, and Experiences (PRAPARE) is a tool used nationally by providers and is designed to be integrated with existing systems. The goal is to both better meet patient needs related to social determinants of health and to collect and apply data to improve health care systems.

TOOL: Enabling Services Data Collection Implementation Companion


Enabling services are non-clinical services that improve access to care, such as transportation and interpretation. It is often hard for health centers to illustrate the importance of these services. This tool, created by the Association of Asian Pacific Community Health Organizations (AAPCHO), along with partners, provides a guide for clinics to collect data on enabling services "to demonstrate to payers and policymakers the value these services bring."
