Outreach Reference Manual 3rd Edition
HOP is continually updating and creating new chapter for the Outreach Reference Manual. While the original manual was geared specifically toward farmworker health outreach programs, HOP’s new Outreach Reference Manual chapters provide direction for how to most effectively use outreach to increase access to and utilization of comprehensive primary health care services across many underserved communities.
Program Planning and Evaluation Chapter
Planning and evaluation are key to effective, efficient outreach programs. This chapter guides readers through participatory planning methodologies and the development of three key frameworks: 1) a logic model that articulates what the program’s impact will be, 2) a work plan that outlines how the program will achieve that impact, and 3) an evaluation plan that outlines how the program will evaluate if it was successful.

Clinical Outreach Chapter
Clinical outreach is defined as the coordination and provision of clinical services in an outreach setting. It is an approach to meet individuals and communities where they are and to extend care in settings that best fit their needs and the context of their lives. The purpose of this chapter is to support health centers by providing a practical framework and guidance on undertaking clinical outreach efforts. It explores the different facets of clinical outreach and how it can be planned and implemented.

The Role of Outreach in Care Coordination Chapter
Underlying many new models of care is the concept of care coordination, which emphasizes collaboration between providers to increase quality of care and ultimately improve patient outcomes as well as reduce the cost of health care. The purpose of this chapter is to support health centers with improving or expanding their care coordination efforts. This chapter makes the case for integrating outreach workers into care coordination teams and shares examples of how health centers can accomplish this.
