Program planning and evaluation is key to conducting effective outreach. These tools will support you in setting measureable goals and objectives, defining your targets and benchmarks, establishing organizational and individual accountability, and understanding whether your outreach goals were accomplished.

MAP: 2015 Health Center Program Grantee Profiles
HRSA generates an interactive map annually through which core sets of data for each health center program grantee are made publicly available.
