Program planning and evaluation is key to conducting effective outreach. While each health center may have a unique approach to outreach, HOP recommends three general resources critical to the planning and evaluation process: a logic model, a program plan, and an evaluation plan. These tools will support you in setting measureable goals and objectives, defining your targets and benchmarks, establishing organizational and individual accountability, and understanding whether your outreach goals were accomplished.
