Outreach Program Planning and Evaluation - Starter Kit

Program planning and evaluation is key to conducting effective outreach. While each health center may have a unique approach to outreach, HOP recommends three general resources critical to the planning and evaluation process: a logic model, a program plan, and an evaluation plan. These tools will support you in setting measureable goals and objectives, defining your targets and benchmarks, establishing organizational and individual accountability, and understanding whether your outreach goals were accomplished.

MAP: 2015 Health Center Program Grantee Profiles

HRSA generates an interactive map annually through which core sets of data for each health center program grantee are made publicly available. Data sets may be sorted by health center, by state or nationally. Available information includes, but is not limited to, patient demographics, services provided by the health center, costs, and revenue.
