Medical-Legal Partnerships for Farmworker Health and Legal Service Organizations

Farmworker patients often experience the negative health impacts of the social determinants of health such as poor housing, employment issues, violence, and not being eligible for health and social programs. Migrant health centers provide critically needed, comprehensive health services to farmworker populations. This includes medical and dental services, health education, case management, transportation, and interpretation. Medical-Legal Partnerships (MLPs) are increasingly working to address the social determinants of health by addressing both the health and the legal needs of patients. According to the National Center for Medical-Legal Partnerships, the MLP model is carried out in a variety of ways and includes three main components: legal assistance, transforming health and legal institutions and practices, and policy change. Formal MLPs may involve one or more legal organizations wherein legal staff and attorneys work with front-line staff to screen for health-related legal problems related to housing, immigration, disability, employment instability, and additional situations that lead to stress and cause or exacerbate health problems. Informal MLPs often include legal representatives participating on health center boards or as members of a coalition serving as a legal resource for health centers. Medical-legal partnerships can be an integral part of comprehensive care for farmworkers and their families by addressing the many social and legal problems they face.
Representatives from health centers and legal agencies serving migrant and seasonal farmworkers served as panelists for the webinar.

The Role of Outreach in Emergency Preparedness for Community Health Centers

Emergency Preparedness defines a community’s readiness to react constructively to natural and human-made threats in order to minimize harm to public health. Community Health Centers have an important role in emergency preparedness. As part of the safety net of the health care system, many health centers provide critically needed health and enabling services and are often the sole provider of these services in their communities. As outreach workers are often the link between the communities and the health centers, they can assist health centers to plan for and manage emergencies in their communities and ensure that these plans and activities are responsive to the underserved and vulnerable populations in their communities. The purpose of this webinar is to share strategies and provide practical advice around the role of outreach workers in emergency preparedness. Topics include: an overview of emergency preparedness, its importance for community health centers and vulnerable populations, the role of outreach workers in emergency preparedness, what initiatives and strategies are being used, and a review of the recently published Emergency Preparedness Tip Sheet for Community Health Centers.

Reaching Underserved Populations Through Mobile Health Care: Lessons Learned From the Field

Mobile health, which can include mobile clinics, mobile vans and other methods used to provide medical or dental services outside a clinical setting, is one of the key ways that many health centers enhance their ability to bring critical health services directly to patients where they live and work. Throughout the country, medical and dental providers are using mobile health as a way to reach underserved populations in urban, suburban and rural communities. Mobile health offers flexibility and efficiency for providers when reaching out and delivering services to vulnerable populations. It also helps overcome transportation barriers that would otherwise prevent patients from accessing health care services.
Mobile health can be an effective way to deliver health care services, but it can also present many challenges. The purpose of the webinar is to exchange experiences, challenges and practical advice around some key areas for implementing effective mobile health services. Topics will include: services provided, staffing, scheduling and follow-up, and collaboration with other agencies. The webinar will also explore how mobile health can help address transportation barriers to accessing health care services, increase access to and utilization of primary care services, and meet the HRSA program requirement regarding accessible hours of operation and service delivery locations. Panelists participating in the webinar include representatives from health centers and other programs serving both migrant and seasonal farmworkers and homeless individuals.

The Role of Outreach Programs in Improving Health Outcomes

Farmworkers have a disproportionately higher disease burden compared to the general population, yet they are significantly less likely to use preventive health services. Health outreach programs play a critical role in extending and improving the health care provided to farmworkers. Fifty percent (50%) of respondents to Health Outreach Partners 2010 national needs assessment said that health education is one of their most common outreach activities, and nearly 40% reported basic health screenings as a key outreach activity. In this event, we will explore the unique strengths of health center outreach programs in improving patient care and health outcomes. Administrators, outreach staff, and clinicians alike are invited to examine specific outreach practices that improve health outcomes while also discussing approaches to measuring them.

The Role of Outreach in the Patient-Centered Medical Home

Because of escalating medical care costs and national health care reform, community health centers are receiving well-deserved praise and attention for their cost-effective and high-caliber health services. Community health centers are particularly well suited to adopt the Patient Centered Medical Home (PCMH) model, a health delivery system which incorporates finely coordinated medical care that responds to the needs of the “whole person.†Patients from underserved and vulnerable populations can especially benefit from the personalized and detailed care found within the PCMH model. Community health workers, outreach workers, promotores de salud, and health educators are uniquely positioned to identify and respond to the various medical and social service needs of different populations in their area, which fuels a well-coordinated and successful PCMH. Panelists will examine how outreach programs are integral to the PCMH team.
