National Outreach Guidelines

Outreach is a critical function of health centers serving underserved populations. In 2000, Health Outreach Partners (HOP) developed its Farmworker Outreach Program Guidelines based on 30 years of experience providing direct outreach services to migrant and seasonal farmworkers. In 2012, HOP collaborated with four national partner organizations (the National Association of Community Health Centers, National Health Care for the Homeless Council, the Association of Asian Pacific Community Health Organizations and Migrant Health Promotion) and an Advisory Panel made up of representatives from community health centers and other support organizations across the country to update and expand on those guidelines, creating the current “National Outreach Guidelines for Underserved Populations.”

Community Health Needs Assessment Toolkit

Health Outreach Partners (HOP) is proud to present its toolkit titled “Community Health Needs Assessment: A Comprehensive Guide to Understanding and Responding to the Needs of Your Community”. The toolkit provides a wealth of information, practical tools, and strategies to support organizations in their efforts to meet the health needs of underserved populations. The easy-to-use toolkit guides the user through the 5 key steps of the needs assessment process, and it is intended for health centers, Primary Care Associations, safety net providers, and local and state agencies.

Collaboration Toolkit for Community Health Organizations

Health Outreach Partners (HOP) proudly presents its collaboration toolkit titled “Taking Collective Action to Support Health Access: A Collaboration Toolkit for Community Health Organizations.” The purpose of the toolkit is to provide the user with guidance and practical tools on engaging in collaborations to increase their collective impact. The toolkit is designed to include a broad application of the term collaboration, consisting of coalitions, partnerships, and referral networks. Whether you are starting from scratch to build relationships with other individuals, or are looking for strategies to expand your current partnerships, this toolkit can be referenced as one comprehensive resource or any section can be used on its own.

Transportation and Health Access: A Quality Improvement Toolkit

Health Outreach Partners presents its newest toolkit titled “Transportation and Health Access: A Quality Improvement Toolkit.” The toolkit is an easy-to-use, practical guide to assist health centers with assessing the scope of the problem and finding solutions to missed medical appointments due to transportation barriers. The toolkit guides the user through the two key phases of the quality improvement process: Needs Assessment and Plan-Do-Check-Act (PDCA) cycle, a continuous quality improvement process. Each section contains an overview of the concepts and sample tools. The tools are designed to be a starting point, and can be customized as needed to align with the specific context and resources of each health center.

Outreach Reference Manual

HOP’s Outreach Reference Manual is a comprehensive reference for outreach programs to improve program effectiveness and sustainability. Tackling such topics as cultural competency, health education, recruitment and retention, needs assessment, planning, and evaluation, the variety of information included is not just relevant for outreach staff, but for everyone working to improve access in the community.

Outreach Reference Manual, 3rd Edition

Planning and evaluation are key to effective, efficient outreach programs. This chapter guides readers through participatory planning methodologies and the development of three key frameworks: 1) a logic model that articulates what the program’s impact will be, 2) a work plan that outlines how the program will achieve that impact, and 3) an evaluation plan that outlines how the program will evaluate if it was successful.

Clinical outreach is defined as the coordination and provision of clinical services in an outreach setting. It is an approach to meet individuals and communities where they are and to extend care in settings that best fit their needs and the context of their lives. The purpose of this chapter is to support health centers by providing a practical framework and guidance on undertaking clinical outreach efforts. It explores the different facets of clinical outreach and how it can be planned and implemented.

The Role of Outreach in Care Coordination Chapter

Underlying many new models of care is the concept of care coordination, which emphasizes collaboration between providers to increase quality of care and ultimately improve patient outcomes as well as reduce the cost of health care. The purpose of this chapter is to support health centers with improving or expanding their care coordination efforts. This chapter makes the case for integrating outreach workers into care coordination teams and shares examples of how health centers can accomplish this.
