Comprehensive planning is key to the success of an outreach program. It includes considering the outreach program’s whole community and organizational context to create a program that is sustainable, responsive, and effective. This webinar will provide the information and tools needed for outreach programs to engage in a comprehensive program planning process, including the key components of planning an outreach program, the importance of outreach program organizational integration with the broader health center goals, ways to incorporate organizational and outreach priorities into planning, and how to conduct a planning meeting with staff. The incorporation of outreach services into patient centered medical home and team-based health care has been shown to reduce disparities and drive down health care delivery costs by improving access to services. However, in many cases, health centers are unclear on how to most effectively coordinate clinical services with outreach services to achieve full primary-care team integration. This webinar will discuss health center mandated performance measures to illustrate how the integration of outreach services can be beneficial. It will serve as an introduction to the clinical core measures and the skills required of outreach workers to contribute to their completion. The webinar will also provide case studies to show how integration of outreach into primary care is possible.

Recent changes to the interpretation of agriculture in the Migrant Health Program have resulted in new opportunities for Health Centers to increase and broaden the scope of their outreach efforts to agricultural workers in their patient populations.  This webinar will provide an overview of the federal definition of agriculture, including qualifying industries, the classification of agricultural workers, and guidance from the 2015 Uniform Data System for accurate reporting.  Presenters will share resources and tools to assist health center staff in implementing effective processes and procedures in verifying the agricultural worker status of their patients. Participants will have an opportunity to apply the information presented with case studies that illustrate the most common challenges in the verification process. Presented by National Center for Farmworker Health.

In order to establish robust outreach programs, it is critical to establish a vision (for the outreach program) and a process to monitor its progress. A logic model is one of the most common tools used to gain a clear vision for planning and evaluating a program, and an evaluation plan ensures that progress towards that vision can be monitored. In this webinar, presenters will guide participants through the steps in developing a logic model. Participants will learn how to create an evaluation plan to monitor progress towards desired outcomes. This webinar builds on content introduced in an earlier webinar entitled Understanding Comprehensive Outreach Program Planning.
