This resource includes a collection of key resources, lessons learned, and effective strategies identified during our March 2023 Learning Collaboratives to address diabetes among Special and Vulnerable Populations along the HRSA-defined diabetes continuum of care. Health Outreach Partners, the Corporation of Supportive Housing and The National Health Care for the Homeless Council partnered to bring this important resource to health centers.
