The Inclusion Scorecard for Population Health™ (ISPH) is an online, web enabled assessment tool featuring over 80 emerging and best practices that advance health equity.  These have been sourced from publications by the CDC, American Hospital Association, HRSA’s Health Center Program Compliance Manual and the American Medical Association and cross referenced with the national CLAS standards linked to serving diverse patient populations.

The best practices or activities are divided into four focus areas as follows:

Tracking the Metrics of Inclusion for Healthcare: These activities support understanding the demographic profile of the communities served and monitoring indicators for key patient outcomes by key demographic groups as well as the tracking the presence of diverse staff at all levels.

Creating a Culture of Inclusion to Support Health Equity: These activities highlight adopting values, principles, and protocols to support culturally effective and coordinated care tailored to the needs of patients and creating an environment that values and leverages the diversity of.

Managing the Link Between Equity and Quality of Care: These activities center on holding leaders accountable for practices that link quality patient outcomes with health equity and rewarding inclusive leadership and management practices.

Inclusive Community Engagement: These activities center on the health systems engagement with community stakeholders in addressing the social determinants of health, health promotion, and health education to understand and support the needs of the community.

The Scorecard has an integrated knowledge base that can support leaders and staff who are interested in adopting these best practices. Resources include reference material, examples of other systems use of the practice and key implementations strategies.
