National Outreach Guidelines
Outreach is a critical function of health centers serving underserved populations. In 2000, Health Outreach Partners (HOP) developed its Farmworker Outreach Program Guidelines based on 30 years of experience providing direct outreach services to migrant and seasonal farmworkers. In 2012, HOP collaborated with four national partner organizations (the National Association of Community Health Centers, National Health Care for the Homeless Council, the Association of Asian Pacific Community Health Organizations and Migrant Health Promotion) and an Advisory Panel made up of representatives from community health centers and other support organizations across the country to update and expand on those guidelines, creating the current National Outreach Guidelines for Underserved Populations.
