The North Carolina Community Health Center Association proudly presents our 2016 Outreach Workforce Development Webinar Series. This 5-part series, led by Health Outreach Partners (HOP), will cover a variety of topics to support outreach program development. HOP, Migrant Clinicians Network, and the National Center for Farmworker Health will present content and share strategies to help you effectively structure and manage a comprehensive community health outreach program.

Understanding Comprehensive Outreach Program Planning

March 23, 2016

Comprehensive planning is key to the success of an outreach program. It includes considering the outreach program’s whole community and organizational context to create a program that is sustainable, responsive, and effective. This webinar will provide the information and tools needed for outreach programs to engage in a comprehensive program planning process, including the key components of planning an outreach program, the importance of outreach program organizational integration with the broader health center goals, ways to incorporate organizational and outreach priorities into planning, and how to conduct a planning meeting with staff. Presented by Health Outreach Partners

  • Participants will be able to identify the components of planning and how they can be applied to their outreach programs.
  • Participants will identify at least two elements of outreach program integration that can be applied in their own organizations.

Integrating Outreach into Clinical Care Teams

The incorporation of outreach services into patient centered medical home and team-based health care has been shown to reduce disparities and drive down health care delivery costs by improving access to services. However, in many cases, health centers are unclear on how to most effectively coordinate clinical services with outreach services to achieve full primary-care team integration. This webinar will discuss health center mandated performance measures to illustrate how the integration of outreach services can be beneficial. It will serve as an introduction to the clinical core measures and the skills required of outreach workers to contribute to their completion. The webinar will also provide case studies to show how integration of outreach into primary care is possible.

Learning Objectives:

  • Participants will identify effective strategies for rethinking the primary care team.
  • Participants will reflect on the current role of outreach workers within their own primary care setting.
  • Participants will be able to describe the HRSA-established core clinical measures for outreach and clinical care.
  • Participants will analyze the readiness of their own clinical setting to link outreach worker activities with clinical measures.

Migrant and Seasonal Agricultural Worker Verification.

April 20, 2016

Recent changes to the interpretation of agriculture in the Migrant Health Program have resulted in new opportunities for Health Centers to increase and broaden the scope of their outreach efforts to agricultural workers in their patient populations.  This webinar will provide an overview of the federal definition of agriculture, including qualifying industries, the classification of agricultural workers, and guidance from the 2015 Uniform Data System for accurate reporting.  Presenters will share resources and tools to assist health center staff in implementing effective processes and procedures in verifying the agricultural worker status of their patients. Participants will have an opportunity to apply the information presented with case studies that illustrate the most common challenges in the verification process. Presented by National Center for Farmworker Health.

  • Describe the relationship between verification, Uniform Data System reports, and health center funding
  • Identify industries and tasks in agriculture that meet the definition for agricultural worker in the Migrant Health Program
  • Distinguish between migratory and seasonal agricultural workers
  • Utilize acquired knowledge to correctly determine the agricultural worker status of health center patients from case studies

Establishing an Outreach Program Vision and Monitoring Your Progress

In order to establish robust outreach programs, it is critical to establish a vision (for the outreach program) and a process to monitor its progress. A logic model is one of the most common tools used to gain a clear vision for planning and evaluating a program, and an evaluation plan ensures that progress towards that vision can be monitored. In this webinar, presenters will guide participants through the steps in developing a logic model. Participants will learn how to create an evaluation plan to monitor progress towards desired outcomes. This webinar builds on content introduced in an earlier webinar entitled Understanding Comprehensive Outreach Program Planning.

  • Participants will gain the information and tools to create a logic model for their outreach program.
  • Participants will learn how to utilize an evaluation plan to monitor progress towards outreach goals.

Planning for Outreach Program Implementation

Implementing a comprehensive outreach program requires structure that allows all members of the team to understand their roles and responsibilities. Programmatic work plans outline how an outreach team will complete their work by describing exactly what the program hopes to accomplish and the specific steps to do so. Policies and protocols establish important guidelines for carrying out outreach activities and outline specific processes that support implementation. In this webinar, participants will learn how to develop a work plan for their outreach program. Participants will also gain the tools to develop policies and protocols for their program. This webinar builds on planning components introduced in the previous webinar entitled Establishing an Outreach Program Vision and Monitoring Progress.

Learning Objectives:

  • Participants will be able to identify and describe the components of a work plan and create one for their own program.
  • Participants will be able to use the tools presented to develop their own policies and protocols.
  • Participants will understand how all of the components of planning work together to support the implementation of a comprehensive outreach program.
