Health Outreach Partners is excited to announce Pathways to Inclusion is live! Our inaugural e-learning series for community health teams, designed to help better serve individuals with intellectual and developmental disabilities (IDD).
This learning series is supported by the Special Olympics Systems Change for Inclusive Health Subgrant, funded by the Centers for Disease Control and Prevention (CDC).

The learning series is comprised of four training priorities
Outreach and Enrollment
This Outreach & Enrollment training is designed for members of the care team who may work with people with intellectual disabilities in improving their access to affordable health coverage.
Trauma Informed Motivational Interviewing
The Trauma Informed Motivational Interviewing (TIMI) training explores how care teams can infuse the principles of TIMI within their care coordination of patients with intellectual and developmental disabilities.
Social Determinants of Health
This training explores how the social determinants of health impact the health care and health outcomes of people with intellectual and developmental disabilities.
Transportation
This training examines transportation barriers and their impact on both individuals with Intellectual and Developmental Disabilities (IDD) and health centers.
