Health Outreach Partners is honored to support the Health Outreach Partners is honored to support the National Center for Farmworker Health (NCFH) as they mark 50 years of impact. We honor one of our longest-running training traditions, the Regional Stream Forums. First launched in 1987 along the East Coast and later expanded to the Midwest and West, these gatherings united health center staff and partners serving agricultural workers across the country. For more than 28 years, the forums fostered collaboration, provided a space to share best practices, and highlighted new resources to strengthen care for migratory and seasonal agricultural workers. 

In response to the evolving needs of today’s workforce, the Regional Stream Forums have transitioned into the NCFH Agricultural Worker Health Symposia, two annual events designed to deliver focused, high-impact learning for health center staff, board members, and partner organizations dedicated to improving agricultural worker health.

We’re excited to welcome Brian Cresta to Health Outreach Partners’ Board of Directors in 2025!
Brian is a national leader in health and human service programs with decades of experience spanning government, managed care, and innovative food-as-medicine solutions. He currently serves as Senior Vice President of Market & Business Development at Mom’s Meals, leading growth in Medicaid and Medicare programs to help people live healthier lives through nutritious food and compassionate support.

A former Presidential Appointee at HHS and CMS and a past member of the Massachusetts House of Representatives, Brian brings deep expertise, public service, and a passion for equitable care.

We’re honored to have his leadership guiding our mission!

We’re thrilled to spotlight board member Keri V. Castañeda, a powerhouse leader with 20+ years of advancing equity in healthcare and human services.
As Chief Operating Officer at AbilityFirst, Keri leads programs that support 1,600+ individuals with developmental disabilities each year. Her career also includes leadership roles at Planned Parenthood and Ravenswood Family Health Network Health Center, always with a focus on expanding access for underserved communities.
A proud Stanford University alum and Coro Health Leadership fellow, Keri brings both lived experience, growing up in Washington’s Yakima Valley, and a deep commitment to advocacy and inclusion.
When she’s not leading change, you’ll find her hiking, reading, or traveling with her family in Los Angeles.

Join the  Association of Asian Pacific Community Health Organizations (AAPCHO), Health Outreach Partners, MHP Salud, and the National Health Care for the Homeless Council for a webinar that will discuss the workforce skill-building needs at community health centers when collecting data on non-medical factors of health, as identified during listening sessions with community health center staff in 2024. Kevin Luu is a second-year graduate student pursuing his Master of Public Health at San Jose State University. He’s passionate about improving health outcomes for youth and children and building a more equitable future for all. He is making meaningful contributions to our Special Olympics project. 
Outside of his studies, Kevin enjoys playing guitar, trying his hand at stand-up comedy, and relaxing with his favorite streaming shows. 🎸🎤📺
