Safe and accessible transportation is a top concern for older adults, caregivers, and their communities. As adults age, maintaining mobility is fundamental to their health and quality of life.

Join Health Outreach Partners (HOP) for the final webinar in our 3-part Transportation Webinar Series, Addressing the Transportation Needs of Older Adults. We will examine the unique considerations when providing transportation services to older adults, and present a real life example of how a partnership between a local community organization and health care district is tackling these barriers. The National Center for Equitable Care for Elders will explore the impact of transportation barriers on older adults, and the Peninsula Volunteers, Inc. will share their Lyft ridesharing program for older adults living in the Sequoia Health Care District. 

Learning Objectives: By the end of the webinar, participants will be able to…
• Understand the unique transportation needs of older adults
• Identify at least one intervention used to address transportation barriers for older adults
• Describe the role transportation access plays in supporting older adults in accessing chronic disease management, decreasing readmissions and ER visits, and addressing social isolation
