Diabetes Continuum of Care: Understanding Social Determinants of Health and Structural Barriers Affecting Diabetes Care Please join HOP, CSH, and NHCHC on Tuesday, March 7th, 14th, 21st, & 28 at 11am-12pm PT; 2pm-3pm ET for "Diabetes Continuum of Care: Understanding Social Determinants of Health and Structural Barriers Affecting Diabetes Care” The Special and Vulnerable Populations Diabetes Task Force, comprised of 14 National Training and Technical Assistance Partners (NTTAPs), invite you to join a national learning collaborative that takes a deeper dive into issues around systemic, structural, and environmental barriers and strategies for improving diabetes care among people experiencing homelessness, residents of public housing, migratory and seasonal agricultural workers, school-aged children, older adults, Asian Americans, Native Hawaiians, and Pacific Islanders, LGBTQIA+ people, and other health center patients. |
