Expanding the Reach of Care: Utilizing Health Education for Diabetes Prevention and Control.

An estimated 29.1 million people have diabetes in the United States. Although a national health concern, diabetes disproportionately affects the migratory seasonal agricultural worker (MSAW) population. In this workshop, Health Outreach Partners (HOP) and MHP Salud will make the case for utilizing outreach workers and community health workers/promotores de salud to conduct diabetes health education with MSAWs. Through interactive group discussions and activities, participants will learn about the PRECEDE-PROCEED model, understand the Stages of Behavior Change, and identify specific outreach activities in diabetes prevention and control
