Protecting Healthcare for All




In July 2025, Congress passed the mega budget reconciliation bill H.R. 1, colloquially known as The One Big Beautiful Bill Act. This budget reconciliation bill effectively cuts $1.15 trillion dollars over the next decade of funding for health care, food assistance, and other vital social services for Americans across the U.S. Due to these cuts, it's estimated that in total 17 million people will lose their health coverage (1).



Some highlights of the bill that will be implemented are:

  • Medicaid Work Requirements:

    • The work requirement dictates that parents and childless adults must prove that they are participating in eligible “community engagement” activities totaling up to 80 hours a month.

    • We don't yet know what “community engagement” means or what the work requirement will look like once implemented, but we can use the state of Georgia as a case study. Georgia has required work requirements for their Pathways to Coverage 1 Medicaid program for the last two years.

    • Qualification for who will be considered “medically frail” will also take effect. Starting January 1st, 2027 states will start the implementation of this new requirement and fewer people will be deemed medically frail. It's estimated that the new work requirement will be responsible for 7 million Americans losing access to Medicaid (2).



  • More Frequent Medicaid Redeterminations:

    • States will now be required to verify Medicaid eligibility every six months starting January 1st, 2027. This increases the administrative burden of Medicaid enrollers and increases the odds of eligible people losing their coverage due to administrative issues such an address typos, other paperwork errors, or erratic technological glitches. With this new provision, it is estimated that 11.8 million people will lose their health coverage by 2034 (3).



  • Eligibility for Lawful Immigrants:

    • Long before this bill has been passed people in the United States without appropriate documentation had never been able to enroll social service benefits.

    • The new provisions in H.R.1 now takes away health coverage and food assistance from anyone with a lawful status, which are most categories of lawful immigration, such as people granted humanitarian protections (i.e. refugees, people granted asylum, certain survivors of domestic violence, sex or labor trafficking) unless you are a:

      • Citizen and national of the United States;

      • Lawful permanent residents (LPRs also known as green card holders);

      • Cuban-Haitian entrants who meet specific criteria defined in the Refugee Education Assistance Act of 1980;

      • People from Micronesia, Marshall Islands, Palau who live and work in the U.S under the Compacts of Free Association (COFA), also referred as COFA migrants (4).






With all of this we must remember that these changes will also increase the administrative burden on enrollers and staff by tenfold. There is more red tape that must be navigated, leaving more opportunities for administrative errors to be made and eligible Americans getting unenrolled.

In addition to the major cuts happening to health care, H.R.1 also changes the tax rules that will affect how donors can deduct charitable gifts. Some key changes donors must look out for are:

  • Claiming Charitable Deductions:

    • In order to claim a charitable deduction, your total annual giving must now exceed 0.5% of your Adjusted Gross Income (AGI).



  • Credit for Scholarship Granting Organizations:

    • Beginning in 2027, there will be a nonrefundable credit up to $1,700 (or 100% of the gift, whichever is lower), for contributions to K-12 scholarship granting organizations (5).




Now more than ever we must work together to support our most vulnerable populations who will be most impacted. Health Outreach Partners has and is prepared to ensure the health center workforce is prepared for the changes. Our Outreach and Enrollment Curriculum 2.0 2 was designed to reflect the changes once the Affordable Care Act got implemented and we once again plan to respond and update the Curriculum to reflect the changes of H.R.1.
