Pathways to Inclusion is Live!

Health Outreach Partners is excited to announce Pathways to Inclusion is live! Our inaugural e-learning series for California community health teams, designed to help better serve individuals with intellectual and developmental disabilities (IDD).

This learning series is supported by the Special Olympics Systems Change for Inclusive Health Subgrant, funded by the Centers for Disease Control and Prevention (CDC).

The learning series is comprised of three training priorities:

1. Outreach and Enrollment for People with Intellectual and Developmental Disabilities
2. How Social Determinants of Health impact people with Intellectual and Developmental Disabilities
3. Trauma-Informed Motivational Interviewing tools and techniques for individuals with Intellectual and Developmental Disabilities

All of our pre-recorded training videos and presentations are housed on our learning management system, Moodle. To access the learning series in its entirety, please go to healthoutreachpartners.moodlecloud.com

Once you get to Health Outreach Partners' Moodle Site, you will click Create New Account.

After creating your account and using the emailed link to login, you will land at the Moodle Dashboard.

Select Home from the top left side of the screen to see the available courses

Once on the Home page you will be able to Self-enroll in all of the available training modules.

Each module will require an Enrollment Key. The Enrollment Key is listed in the description of each Module.
