Health Outreach Partners created a brief training and technical assistance resource to help community health centers in defining and tracking outreach and enrollment metrics. The resource, developed in response to numerous requests from health centers seeking guidance on evaluation of outreach and enrollment efforts, is intended to provide practical tips and examples. If these tips are followed, it will help health centers establish a solid set of metrics for their outreach and enrollment work.
